Data Collection
The data we collect and store include a patient’s full name, DOB, address, GP, email address and telephone number.
We also collect and appropriately store physiotherapy notes on each patient but keep them in line with general data protection regulation (GDPR).
We do not pass on your data to third parties.
Data Retention
We will keep your data on record for 8 years
These notes are confidential are not shared with anyone except if a request is made by the patient themselves. Please contact Withington physiotherapy to make this request and we shall comply within 1 month.

Tell us how you want to be contacted (e.g. by email, but not by telephone)
Ask to have this information amended, switched off (suppressed) or, in some cases, deleted.
Control what information you wish to receive (text message reminders, special offers emails)
Children (under 16)
Children always should be accompanied by a parent or guardian to appointment and provides consent for their treatment and for data purposes

Any data breaches will be reported to the ICO and the people involved will be informed
